Amenorrhoea

Amenorrhoea means the absence of menstrual periods - either periods never starting by age 16, or periods stopping for three or more months in someone who previously had regular cycles. It has many possible causes, several of them hormonal.

Possible causes

  • polycystic ovary syndrome (PCOS);
  • significant weight loss, excessive exercise, or low body weight;
  • high prolactin levels or thyroid disorders.

Assessment and possible causes

Pregnancy must be considered first when periods stop unexpectedly. Other causes include PCOS, thyroid or prolactin disorders, low energy availability, significant weight change, intensive exercise, stress, menopause, medicines and conditions affecting the ovaries, pituitary or hypothalamus. Assessment uses menstrual and reproductive history, symptoms, medicines and targeted blood tests.

Bone health and fertility

Prolonged low oestrogen can affect bone health, while the underlying cause may also influence fertility. Treatment therefore considers cycle restoration or protection, nutrition, bone risk, pregnancy goals and whether gynaecology or fertility input is needed.

UK-wide video consultations

History and most hormone-result reviews can take place securely by video. Pregnancy assessment, examination, imaging and urgent care are arranged locally. Severe headache with visual change or neurological symptoms requires urgent assessment.
