Conn's Syndrome

Conn's syndrome, also called primary hyperaldosteronism, occurs when the adrenal glands produce too much aldosterone, a hormone that regulates salt and water balance. Excess aldosterone causes high blood pressure and can lower potassium levels. It is an under-recognised but relatively common cause of high blood pressure, particularly when difficult to control with standard medication.

Who should be tested

  • high blood pressure that is difficult to control on multiple medications
  • high blood pressure diagnosed at a young age
  • high blood pressure with low potassium levels

Confirming primary aldosteronism

Screening uses aldosterone and renin interpreted with potassium, medicines, posture and sampling conditions. Abnormal screening is commonly followed by confirmatory testing. Adrenal imaging alone cannot determine which gland is producing excess aldosterone.

Treatment planning

Adrenal vein sampling may be recommended before surgery to distinguish one-sided from two-sided disease. Treatment may involve adrenal surgery or a mineralocorticoid-receptor antagonist, with monitoring of blood pressure, potassium and kidney function.
