Hashimoto's Disease

Hashimoto's thyroiditis is an autoimmune condition where the immune system gradually attacks the thyroid gland, usually leading to an underactive thyroid. Dr Kasi Subbiah provides specialist diagnosis and long-term management for patients with Hashimoto's disease.

Diagnosis and levothyroxine treatment

TSH and free T4 are central to diagnosis. Thyroid peroxidase antibodies can support an autoimmune cause but usually do not need repeated measurement. Ultrasound is considered when there is a lump, goitre or structural concern, rather than for abnormal thyroid function alone.

When hypothyroidism requires treatment, levothyroxine replaces the hormone the thyroid can no longer produce. The dose is individualised and monitored with thyroid blood tests. Food, iron, calcium, antacids and some medicines can affect absorption, so consistent timing is important.

Persistent symptoms and pregnancy planning

If symptoms continue despite thyroid tests in target range, review dose, adherence, interactions and other possible causes rather than automatically increasing thyroid hormone. Thyroid requirements often change early in pregnancy, so pre-conception review and prompt contact after a positive pregnancy test are important.

UK-wide video consultations

History, blood-result interpretation and most dose reviews can take place securely by video. Examination, imaging and urgent assessment are arranged locally when needed.
