Confirming the diagnosis
Symptoms alone do not diagnose testosterone deficiency. Assessment normally requires appropriately timed morning testosterone measurements on separate days, interpreted with sex hormone-binding globulin where relevant, plus LH, FSH, prolactin and other tests guided by the history. Acute illness, poor sleep, obesity and some medicines can temporarily lower testosterone.
Causes, fertility and treatment decisions
Review distinguishes testicular causes from pituitary or hypothalamic causes and considers sleep apnoea, metabolic health, medicines and fertility plans. Testosterone therapy can suppress sperm production and is not a fertility treatment. Any treatment decision balances symptoms, confirmed biochemistry, contraindications and monitoring needs.
