Diagnosis and metabolic assessment

PCOS is diagnosed from a combination of irregular or absent ovulation, clinical or biochemical androgen excess and polycystic ovarian appearance, after excluding other relevant causes. Ultrasound is not required in every case. Assessment may include menstrual history, pregnancy plans, acne or excess hair growth, blood pressure, glucose risk, cholesterol and appropriate hormone tests.

Treatment and fertility planning

Treatment depends on the person’s priorities and may address cycle protection, androgen-related symptoms, fertility, weight and metabolic risk. Options can include lifestyle support, combined hormonal contraception, metformin or fertility referral where appropriate. Treatment is individualised and prescribing is not guaranteed at the first appointment.

History, results and treatment planning can often be reviewed securely by video. Examination, ultrasound, pregnancy care and fertility procedures require suitable local or in-person services.
